What is a Community Health Center?

Health Partners of Western Ohio is an independent, non-profit, community-directed organization serving low income and medically underserved communities. Health Partners is controlled by a volunteer Board of Directors. More than 51% of the Board members are patients of the health center.

Who do we serve?

Our services are available to anyone who wishes to be a patient of Health Partners. We do not turn anyone away for inability to pay.

Do I need an appointment?

It depends. Having an appointment means that your healthcare provider can spend the appropriate amount of time with you in discussing your health concerns. If you need to see a provider immediately, our health centers offer a limited number of same-day appointments. Health Partners – Quick Care located at 1550 N. Main Street in Lima accepts walk-ins throughout the day.

Is there a minimum payment?

Health Partners accepts Medicaid, Medicaid Managed-Care (Buckeye, Paramount, Molina, CareSource, etc.), Medicare, and most commercial insurances. Individuals who have no insurance and an income that falls below 200% of the Federal Poverty Guidelines can receive a discount (Sliding Fee Scale) on services. Health Partners also has Financial Counselors available to help complete Medicaid and Health Insurance Exchange (ObamaCare) applications.

How do I qualify for the Sliding Fee Scale?

Ask to speak with a Financial Counselor. They are available to explain the Sliding Fee Scale and assist patients in completing a screening application. In order to receive a discount, patients are required to provide proof of income.

Who pays for the health care services offered at the health center?

Health Partners’ finance/billing office bills every available resource to pay for services including: Medicare, Medicaid, and Private Insurance carriers. Self-pay patients are also billing based on the Sliding Fee Scale.

What do I do if I cannot pay for my care?

Your ability to pay should not be a barrier to receiving the health care you need. Please ask to speak to a Financial Counselor to see what options are available. You may be eligible to set up a payment plan or we might be able to make arrangements for you to pay for services.

Do you accept private insurance?

Yes. We accept almost all commercial/private insurance plans.

you accept Medicare and Medicaid?

Yes. Health Partners accepts Medicare, Medicaid, and Medicaid-Managed Care plans such as Buckeye, CareSource, Paramount, etc.

What are your policies regarding patient confidentiality?

We are committed to protecting your privacy and have strict policies to ensure patient confidentiality. These policies meet all federal and state laws. If you would like a copy of our privacy practices, please request one during your next visit with us.

What are your sources of funding?

The majority of our funding comes from patient care revenue. Health Partners also receives federal, state and local grant funding and donations. We are designated as a Federally Qualified Health Center by the Health Resources Services Administration. Health Partners leverages every dollar received to make it stretch farther.
